NOTICE OF INTENT TO DESTROY MEDICAL RECORDS AT UNITED MEDICAL CENTERS

Due to storage limitations for old medical records, United Medical Centers will be purging and destroying medical records for the years up to and including 1998, on patients WHO HAVE NOT BEEN SEEN IN OUR CENTERS SINCE 1998.

Records will be purged from United Medical Centers at Lyons and Berthoud. If you would like to have a copy of your 1998 and older records, please call the Medical Record Department at 303.702.5980. You will hear recorded instructions on how to request an official form for releasing those records to you.

The deadline for requesting records is June 1, 2009.
